We are seeking an experienced and motivated Clinical Nurse Manager to lead and support our virtual care team in delivering exceptional patient-centered care in the fields of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and other preventative care programs. This nurse will oversee a team of dedicated Medical Assistants who support patients through virtual care programs while managing a small caseload of your own patients and handling clinical escalations.

What Youll Do
- Lead and mentor a team of clinical MAs delivering virtual patient care.
- Oversee daily team operations, ensuring program goals and patient care standards are consistently met.
- Provide clinical oversight, support escalations, and manage a small panel of patients.
- Track and analyze team metrics including care minutes, call completion, and quality outcomes using Excel and software dashboards.
- Collaborate with cross-functional teams (clinical, enrollment, billing) to ensure smooth patient transitions and care coordination.
- Ensure all care delivery complies with clinical guidelines, documentation standards, and CMS program requirements.
What You Bring
- Active Nursing License (CNA, RN or LPN/LVN)
- 25 years of clinical experience, ideally in geriatrics, family medicine, or chronic care management.
- 2+ years of leadership or supervisory experience within a healthcare or care management setting.
- Strong skills in Excel, performance tracking, and EHR systems.
- Tech-savvy, adaptable, and passionate about improving healthcare through innovation.
- Excellent communicator and coach who can balance empathy with accountability.
