Requirements:
• Active RN license
• Nurse Compact / Multi-state license (required)
• Minimum 3+ years of diabetic care experience
• Proven clinical experience in care coordination or case management (preferred)
• Must provide your own technology (computer, webcam, microphone, speakers, phone, stable internet connection)
• Strong communication, critical thinking, and organizational skills
Responsibilities:
• Provide remote case management for diabetic patients
• Conduct virtual assessments and ongoing monitoring
• Develop, implement, and modify care plans based on patient needs
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Document all interactions and updates in accordance with facility standards
• Ensure compliance with state regulations, professional guidelines, and organizational policies
• Maintain confidentiality and patient data security in a remote environment
