Empowered Staffing is partnered with a skilled nursing company, and, due to growth, they are seeking a compassionate Occupational Therapist who wants to make a positive impact on residents’ lives. In this role, you’ll support individuals in performing daily activities while working closely with the therapy team to deliver exceptional, patient-centered care.
Responsibilities:
- Evaluate residents upon admission and as needed to determine functional deficits and therapy needs.
- Develop individualized treatment plans with measurable goals.
- Provide skilled occupational therapy interventions, including ADL retraining, upper extremity strengthening and ROM, fine motor coordination, cognitive and perceptual training, adaptive equipment training, energy conservation techniques, and safety/fall prevention strategies.
- Complete accurate and timely documentation in accordance with Medicare Part A and Part B regulations, Illinois Department of Public Health (IDPH) requirements, and CMS guidelines.
- Participate in care plan meetings and interdisciplinary team discussions.
- Provide family education and discharge planning support.
- Supervise Occupational Therapy Assistants (COTAs) as required.
- Contribute to QAPI initiatives and performance improvement activities.
Qualifications:
- Graduate of an accredited Occupational Therapy program.
- Current Illinois Occupational Therapist license in good standing.
- NBCOT certification required.
- Previous experience in skilled nursing or post-acute setting preferred.
- Strong documentation and communication skills.
- Ability to work collaboratively with nursing, therapy, and physician teams.
