Schedule: Monday - Friday

· Patients per day: 11-16 patients per day depending on appointment length
· Patient population/age: Adult patients

· EMR: Cerner
· Hospital bed size: 25 beds
· Special procedures:
· Skills required: Med Checks and Med Checks with therapy

· Board Certification: required
· State license required: required
· DEA: finding out

· Minimum experience required: at least 1 year

Credentialing Timeframe: 60 days

Willing To License?: YES (IMLC preferred or providers that qualify for the WV temp license)
