Field LVN - Home Health (PRN & Full-Time Schedules Available)

Job DescriptionTricare Home Health Services Inc. is seeking motivated and compassionate Licensed Vocational Nurses (LVNs) to join our home health team. We’ve proudly served San Antonio families since 1996, and we’re continuing to grow.

We offer per-visit pay for all LVN field positions. Whether you want the flexibility of PRN work or a steady caseload, we have opportunities for you.

What You’ll Do

As a Field LVN, you will provide skilled nursing care in patients’ homes, including but not limited to:

• Medication management & teaching
  
  
• Diabetic care & education
  
  
• Wound care & dressing changes
  
  
• Post-surgical care
  
  
• Patient & family teaching for disease management
  
  
• Monitoring vital signs and patient progress
  
  

You’ll work closely with our RN Case Managers to ensure each patient receives the highest quality of care.

Qualifications

• Current Texas LVN license (required)
  
  
• At least 1 year of clinical experience preferred (home health experience a plus, but training available)
  
  
• Reliable transportation and valid driver’s license
  
  
• Ability to work independently and manage time effectively
