Position Overview

The Community Health Navigator (CHN) oversees AHI’s Afghan Health Coalition, coordinating workshops and community discussions, managing program reporting, and building stronger connections between Afghan women, healthcare providers, and community systems. This role ensures maternal health navigation is delivered in a culturally and linguistically responsive, trauma-informed way that advances trust and equity in healthcare.


Key Responsibilities


Program Implementation & Supervision

  • Coordinate workshops on prenatal, postpartum, and overall maternal well-being.
  • Manage distribution of program supplies, incentives, and resources.
  • Oversee day-to-day operations, ensuring alignment with program deliverables.

Healthcare Provider & Stakeholder Engagement

  • Act as a liaison between clients, healthcare providers, and community partners.
  • Support provider-cultural competency efforts through partnerships and training.
  • Represent AHI and the Afghan Health Coalition in meetings with funders, providers, and stakeholders.

Client Navigation Support

  • Oversee case management documentation and referrals for client services.
  • Facilitate timely access to transportation, interpretation, childcare, and other supports.
  • Ensure delivery of linguistically appropriate and trauma-informed support.

Data Management & Reporting

  • Maintain systems for tracking engagement, attendance, and outcomes.
  • Provide reports and program updates to the Program Director.
  • Collaborate with partners to gather stories, photos, and multimedia content.

Community Engagement & Advocacy

  • Facilitate discussions with Afghan women to identify needs and co-design strategies.
  • Ensure client feedback informs program design and delivery.
  • Support AHI in building trust-based relationships with participants.

Additional Duties

  • Participate in staff meetings, trainings, and professional development.
  • Represent AHI at community events and coalition activities.
  • Perform other duties as needed to advance maternal health equity.


Qualifications


Required

  • Associate’s or Bachelor’s degree in Public Health or Social Services.
  • At least 1–2 years of program or project coordination experience, ideally in health, maternal health, or social services.
  • Strong skills in community outreach, facilitation, and cross-cultural communication.
  • Experience with data management systems and reporting for health or social programs.
  • Problem-solving skills to address barriers clients face in accessing healthcare.
  • Excellent English communication skills (speaking, reading, writing).
  • Strong interpersonal and organizational skills.
  • Knowledge of Afghan cultural values and lived community experience.
  • Bilingual/Trilingual (Dari, Farsi, Pashto, and English).
  • Field-based community experience.
  • Own means of transportation and flexible availability.

Preferred

  • Experience working with immigrant/refugee or diverse communities.
  • Knowledge of maternal health disparities and culturally responsive health promotion.


Compensation

  • Hourly Rate: $23.33–$27.50
