BayCare HomeCare is seeking a Travel Occupational Therapist for a 13-week Home Health assignment in the Tampa area. Join one of the most recognized healthcare systems in Florida while helping patients regain independence and improve quality of life in the comfort of their own homes.

This position offers a diverse caseload of adult and geriatric patients, providing opportunities to work with orthopedic, neurological, cardiac, pulmonary, and post-surgical populations throughout the Tampa Bay community.

Patient Population

Treat a variety of diagnoses including:

• Orthopedic injuries and joint replacements
• CVA / Stroke recovery
• Cardiac conditions
• COPD and CHF
• Post-surgical patients
• Parkinson's Disease
• Falls and balance deficits
• General weakness and deconditioning
• ADL and functional mobility training

Responsibilities

• Complete occupational therapy evaluations and treatments in the home setting
• Develop and implement individualized treatment plans
• Improve patient safety, independence, and functional performance
• Educate patients and caregivers on adaptive techniques and equipment
• Collaborate with physicians, nursing, PT, SLP, and interdisciplinary team members
• Complete timely documentation and OASIS assessments as required
