We are seeking an experienced and compassionate Nurse Practitioner to provide primary care services to residents. This role offers autonomy, flexibility, and the opportunity to build real continuity of care without the demands of nights, weekends, or hospital rounding.

Responsibilities:

  • Conduct routine and follow-up visits for assisted living residents

  • Perform comprehensive assessments, medication reviews, and exams

  • Manage chronic and acute conditions

  • Develop and update care plans in collaboration with interdisciplinary teams

  • Order and interpret labs and diagnostic tests

  • Prescribe medications within scope of practice

  • Educate residents and families on health management and prevention

  • Maintain accurate documentation in eClinicalWorks

  • Participate in quality and clinical improvement initiatives

Schedule & Work Environment:

  • No nights

  • No weekends

  • Assisted living setting

  • Manageable daily patient load

  • Strong clinical and administrative support

Compensation & Structure:

  • $135,000–$160,000 annually depending on schedule and experience

  • W2 or 1099 options available

  • Travel reimbursement included

  • Flexible scheduling options

Qualifications:

  • Master’s degree from an accredited Nurse Practitioner program

  • Active New York State NP license

  • DEA registration

  • Eligible for Medicare and Medicaid billing

  • Minimum of 3 years clinical experience preferred

  • Geriatric, long-term care, or primary care experience strongly preferred

  • Strong clinical judgment and patient-centered approach
