Position Description:
The Global Leasing program provides targeted assistance to Trueblood-eligible individuals with acute and complex behavioral health needs to obtain and maintain stable housing, which will ultimately address the interrelation of behavioral health concerns and housing stability. To achieve this work, the Coordinator will have two primary scopes of work: 1, home and community-based supports to eligible individuals- which includes coaching, advocacy, information and referral, linking and coordinating, and ongoing supports; and 2, identifying long term housing opportunities in the community by conducting outreach to private market, and scattered-site landlords in an effort to obtain multiple global leasing opportunities. The Coordinator utilizes both of these areas of work to create a match between the eligible individual and an appropriate housing unit- while providing ongoing tenancy supports as needed (and for as long as the individual remains eligible) so that the individual can remain stably housed.
Essential Job Functions:
- Work closely with FHARPS team members to identify eligible Trueblood participants for Global Lease placements
- Develop and revise individualized housing plans based on the housing preferences of the participants
- Working with participants to develop short and long-term measurable goals with financial stability and housing sustainability as the top priority
- Providing service-planning support and participate in multi-disciplinary team and team planning meetings
- Coordinating and linking participants to community support services i.e. primary care provider, behavioral health services, etc.; substance abuse treatment, including those supports designed to obtain and maintain independent living, SSI/SSDI, SOAR, Supported Employment programs, healthcare for workers with disabilities (HWD).
- Providing entitlement assistance (including obtaining documentation, navigating and monitoring the application process, and coordinating with the entitlement agency)
- Providing supports to assist participants in the development of independent living skills
- Coordinating with participants to review, update and modify their housing support plan and crisis plan
- Connect participant to resources that will assist in being a good tenant
- Working with private market, scattered-site landlords to grow and foster opportunities for housing, specifically global lease opportunities. Engage in all processes required to secure global leases, including marketing, negotiating and developing contracts and meeting with legal representatives as needed.
- Provide transitional assistance to participants who wish to enter or exit a global lease placement.
- Act as a liaison between both FHARPS staff as well as landlords and property owners, to promote the success of individual participants and their transition to the global leasing program.
- Coordinate placements and provide ongoing supports by being present within the global leased units on a regular basis and having flexible availability for all tenants.
- Engage in ongoing collaboration with property managers/landlords regarding tenancy needs, repairs, safety and day to day operations of each global lease unit.
- Act as a mediator between housemates, develop behavior plans and contracts, and report to Property Managers/Landlords as needed.
Non-essential Job Functions:
- Supports agency mission, goals, and objectives.
- Attend staff and planning meetings as scheduled.
- Maintain communication and cooperative working relationships both within the agency and community at large, while maintaining a high degree of confidentiality.
- Other duties as assigned.
Education, Licensing and Experience:
- Bachelor degree in Mental Health, Social Work, Psychology, or Human Services preferred, but not required.
- Approximately +5 years of experience in a social service field, within a supportive housing related program, with mental illness/chemical dependency preferred.
- Experience in marketing, contract development and contract writing skills preferred
