Job Title: LPN/LVN – Long Term Care (LTC)
Location: Falmouth, MA
Job Type: Contract
Duration: 10 Weeks
Schedule: 40 hours/week | Mixed Shifts (7:00 AM – 3:30 PM & 3:00 PM – 11:30 PM)
Weekend/Holiday Requirement: Every other weekend and holiday

Position Overview

We are seeking a dedicated Licensed Practical Nurse (LPN/LVN) to provide high-quality nursing care in a Long-Term Care (LTC) setting. This role involves direct patient care as well as supervision of nursing assistants, ensuring compliance with all applicable regulations and maintaining the highest standards of resident care.

Key Responsibilities

  • Provide direct nursing care to residents in accordance with care plans and physician orders
  • Supervise and direct nursing assistants in daily care activities
  • Coordinate and plan shift activities with nursing staff and support personnel
  • Complete and maintain accurate documentation for admissions, transfers, and discharges
  • Receive and transcribe physician orders into patient records and care plans
  • Maintain detailed and accurate nursing notes reflecting patient care and responses
  • Encourage physician engagement in reviewing treatment plans and documentation
  • Ensure compliance with federal, state, and facility regulations
  • Promote a safe, clean, and supportive environment for residents

Required Education

  • Graduate of an accredited nursing program or approved LPN program

Required Certifications & Licensure

  • Active, unencumbered Massachusetts LPN license
  • Basic Life Support (BLS) / CPR and AED certification

Required Skills & Experience

  • Minimum 1 year of experience in a Long-Term Care (LTC) setting
  • Strong knowledge of nursing practices and patient care standards
  • Ability to supervise and lead nursing assistant staff
  • Excellent communication and documentation skills
  • Ability to provide age-appropriate care to residents
  • Strong organizational and time management skills
  • Proficiency in English (reading, writing, speaking, and comprehension)
