Guided by the primary care operations team, the Primary Care Patient Care LPN collaborates closely with our Primary Care Providers to ensure safe, timely, effective, efficient, and equitable patient-centered care. The Primary Care Patient Care LPN will work alongside patients, providers, and other clinical team members to promote quality outcomes for all individuals. Responsibilities will be carried out in alignment with our mission, values, and service standards.

Position Details:

  • Experience: A minimum of 6 months of clinical healthcare experience, ideally in a primary care setting.
  • Required Education: Completion of an accredited practical nursing program.
  • Licensure: Must hold an active LPN (Licensed Practical Nurse) license in the state of practice.
  • Additional Qualifications: Ability to collaborate effectively as part of a team, with strong written and verbal communication skills. A customer service mindset, excellent organizational and interpersonal abilities, a flexible and positive demeanor, and a keen attention to detail and accuracy. Must be self-motivated and capable of performing tasks independently within the scope of practice with minimal supervision.
