LPN Clinical Coordinator (Full-Time, Exempt)
Falcons Landing is seeking a motivated, experienced, and compassionate LPN Clinical Coordinator to join our Healthcare team. This is an excellent opportunity for a skilled nurse ready to take on a leadership role within a highly respected, not-for-profit Life Plan Community.

Key Responsibilities
- Oversee daily clinical operations within the Memory Care Unit
- Supervise and support nursing staff, including hiring, training, and performance management
- Ensure compliance with Virginia Department of Social Services (DDS) regulations
- Coordinate care delivery and maintain accurate clinical documentation using EMR systems
- Promote a positive, collaborative, and high-performing team environment
Qualifications
- Active Licensed Practical Nurse (LPN) license in Virginia required
- Prior leadership or management experience in a healthcare setting preferred
- Experience in assisted living, memory care, or long-term care
- Familiarity with PCC and electronic medical records (EMR) systems
- Proficiency in Microsoft Office applications
- Strong organizational, communication, and time management skills

Falcons Landing was completed in 1996 and the community is everything the Founders dreamed it would be... and more! We are proud to say we have been serving our residents for more than 27 years.
