• Evening Shift: 3:00 PM – 11:30 PM
• 5x8-Hour shifts
• Contract Duration: 8 Weeks
• Every other weekend required

Job Summary

The LPN/LVN – Long Term Care (LTC) provides direct patient care in a skilled nursing facility, supporting both long-term care and rehabilitation patients. This role involves assisting with daily care, monitoring patient conditions, and collaborating with the healthcare team to ensure high-quality patient outcomes.

Key ResponsibilitiesPatient Care

• Provide direct nursing care under the supervision of an RN
• Monitor patient conditions and report changes promptly
• Perform blood glucose monitoring and related clinical tasks
• Assist with Activities of Daily Living (ADLs)

Clinical Support

• Support care for long-term care and rehabilitation patient populations
• Float between units within the facility as required
• Maintain patient safety and quality standards

Documentation & Systems

• Maintain accurate and timely patient records
• Utilize EMR systems (e.g., Epic)

Compliance & Safety

• Follow facility policies and regulatory requirements
• Ensure infection control and patient safety protocols

Required Qualifications

• Active LPN/LVN license (New York)
• Minimum 1 year of experience in a Long-Term Care (LTC) setting
• BLS (Basic Life Support) certification
• Experience in skilled nursing facility or nursing home environment
