The Psychiatric Nurse Practitioner (PMHNP) provides direct mental health and substance use care under the supervision of a psychiatrist. This includes assessment, diagnosis, treatment planning, and medication management for patients at Medical Associates Plus (MAP), including those served through the Skilled Nursing Facility (SNF) Behavioral Health Program. The SNF Program Coordinator oversees the operational, clinical, and administrative aspects of behavioral health services delivered in skilled nursing facilities, ensuring compliance with federal, state, and FQHC.
Responsibilities and Duties
Clinical Care
- Conduct psychiatric evaluations and manage treatment plans for behavioral health and substance use patients.
- Prescribe medications, order labs/imaging, and interpret results within scope of licensure and MAP protocols.
- Collaborate with psychiatrists and multidisciplinary teams to ensure integrated, patient-centered care.
- Refer patients for higher levels of care as needed and participate in emergency response by MAP policy.
- Educate patients and families on mental health conditions, medications, and care options.
- Ensure timely and complete documentation in the electronic health record.
- Comply with HIPAA, OSHA, and MAP standards, including adherence to GFE and SFS protocols.
- Participate in quality assurance, performance improvement, and peer review activities.
- Promotes the principles of healthcare integration and the Patient-Centered Medical Home.
Program Coordination – Skilled Nursing Facilities (SNF)
- Develop and manage SNF-specific behavioral health workflows, policies, and training materials.
- Coordinate referrals, admissions, and scheduling for SNF BH patients in collaboration with the case manager.
- Serve as liaison to SNF providers, case managers, and staff to facilitate smooth care delivery and follow-up.
- Conduct telehealth case reviews and attend virtual multidisciplinary meetings with SNF staff.
- Triages urgent behavioral health needs and oversees crisis intervention planning for SNF patients.
- Maintain SNF caseloads and monitor patient progress; adjust care as needed for symptom improvement.
- Collaborate with billing to resolve issues and ensure timely payment for services.
- Develop psychoeducational resources for patients and SNF staff, including symptom management and brief therapeutic techniques.
- Represent MAP in outreach efforts related to expanding behavioral health services in local nursing homes and similar facilities.
Qualifications
Knowledge, Skills & Abilities
- Advanced clinical knowledge of psychiatric and substance use disorders.
- Strong understanding of behavioral health service delivery in FQHC and SNF settings.
- Ability to work independently within the NP scope of practice and consultation when needed.
- Skilled in crisis management, telehealth coordination, and patient-centered communication.
- Proficient in EHR systems and behavioral health documentation.
- Culturally competent with ability to work with diverse populations.
- Demonstrated leadership, organizational, and interpersonal skills.
Education:
- Graduate from an accredited PMHNP program.
- Licensed as a Psychiatric Mental Health Nurse Practitioner in the state of Georgia.
- DEA registration and board certification required.
Experience:
- A minimum of two (2) years of psychiatric practice is preferred, ideally in long-term or community-based care.
- Experience with SNF populations, program management, or care coordination strongly preferred.
Physical Requirements & Working Conditions
- Must be able to sit, stand, walk, bend, and lift up to 25 pounds.
- Regular exposure to patients in outpatient and virtual SNF environments.
- May involve extended hours or after-hours consultation.
- Vision and hearing must be adequate to perform clinical duties, with or without assistive devices.
