Position Overview:

The Intake and Referral Specialist is a key member of our team, responsible for managing and processing patient referrals and facilitating smooth transitions into our home health and hospice services. This role requires exceptional communication skills, a strong attention to detail, and the ability to work collaboratively with patients, families, physicians, and healthcare providers.


Responsibilities:

  • Review and process patient referrals, ensuring all necessary information is complete and accurate for timely admission into home health or hospice services.
  • Collaborate with physicians, hospitals, skilled nursing facilities, and other healthcare providers to ensure smooth transitions for new patients.
  • Verify patient eligibility and insurance benefits, ensuring all required documentation and approvals are in place.
  • Work closely with the Transitions of Care Team, clinicians, and management to ensure timely and efficient service delivery.
  • Communicate with insurance companies, referral sources, and community partners to resolve any issues or delays in the referral process.

Requirements:

  • Knowledgeable regarding Medical Terminology and Health Care service delivery.
  • Demonstrated agility in use of EMRs or similar IT application mastery.
  • Experience in customer service.
  • Experience in Home Health and Hospice Services or related health care service.
  • A combination of education and experience that is determined to be equivalent to the above listed education and experience requirements.
