Responsibilities:

Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions

Develop and implement individualized care plans in collaboration with the healthcare team

Participate in patient assessments, vital sign checks, and reporting changes in patient condition

Collaborate with RNs, therapists, and other healthcare professionals to ensure seamless patient care

Maintain accurate and confidential patient records, as well as adhere to all HIPAA regulations

Develop and maintain strong relationships with patients, families, and colleagues to ensure a positive and supportive environment
Requirements:

Current LPN license in the state of Pennsylvania

Basic Life Support (BLS) certification
