Location: Mantey Heights Rehabilitation and Care Center, Grand Junction, CO

Type: Skilled Nursing Community

Compensation: $40/hr - $50/hr based on experience - Available for Full-Time, Part-Time, and PRN positions

Role Overview

The Physical Therapist will implement personalized physical therapy programs, which include assessment, treatment planning, execution, education, and communication, aimed at helping patients achieve their functional goals. This role also involves fostering collaborative relationships with other healthcare professionals and support staff.

Qualifications

The following qualifications are essential for this position:

  • Knowledge and skills to address the physical and developmental needs of the patient demographic served.
  • Ability to maintain composure during emergencies or crises.
  • Graduation from a physical therapy program accredited by the American Physical Therapy Association (APTA).
  • A minimum of one year of experience as a physical therapy aide.
  • Current licensure from the state Board of Physical Therapy Examiners.

Key Responsibilities

To excel in this role, the individual must effectively perform the following functions:

  • Deliver direct patient care, assess functional needs and outcomes, and collaborate with other specialists. Adjust care plans based on patient evaluations to ensure optimal results.
  • Develop and implement care plans based on comprehensive assessments, involving significant others when appropriate.
  • Conduct therapy practices in accordance with established care standards.
  • Guide support staff in achieving patients’ functional objectives.
  • Communicate effectively with colleagues to ensure positive patient outcomes.
  • Adhere to hospital and departmental policies, quality improvement initiatives, safety protocols, and infection control standards.
  • Document evaluations, treatment goals, and plans, ensuring regular updates and maintaining necessary prescriptions and signatures.
  • Promote professional growth for oneself and others through participation in educational programs, in-service meetings, and workshops. Stay informed about current professional literature.
  • Provide leadership and guidance to staff, serving as a resource to enhance the visibility of physical therapy within the community.

  • Ability to read, write, and communicate effectively in English to meet job requirements.
  • Capacity to make independent decisions and solve practical problems, interpreting instructions in various formats.
  • Skill in active listening and evaluating social situations objectively and fairly. Ability to work independently with integrity and flexibility while collaborating with residents and staff.
  • Compliance with community policies regarding workplace safety, including infection control and emergency protocols.
  • Adherence to all organizational policies related to resident rights, abuse prevention, and confidentiality (HIPAA).
  • Familiarity with the Employee Handbook and compliance with its policies.
  • Willingness to perform other related duties as assigned.
