Job Overview
Specialty: Hospice RN
Schedule: 5x8s (Day Shift)
Shift: 7:00 AM – 3:00 PM (Monday–Friday)
Contract Length: 14 weeks
Start Date: ASAP
Guaranteed Hours: 40 per week
Setting: Home Health / Hospice
Responsibilities
- Provide hospice and palliative care in home and community settings
- Perform patient assessments and develop care plans
- Manage symptoms and provide comfort-focused care
- Educate and support patients and families
- Coordinate with interdisciplinary hospice team
- Complete documentation and maintain compliance
Requirements
- Active RN license (WA or Compact)
- BLS certification required
- Minimum 1 year of hospice experience required
- Home Care Home Base (HCHB) experience required
- Must have reliable transportation
Additional Details
- 1:1 patient care model (up to ~6 visits/day)
- No on-call requirement
- Mileage reimbursement provided
- Mix of field visits and office-based work
- Epic / HCHB charting
Hospice experience and HCHB required. Must have reliable transportation. Comfortable working independently in the field and managing patient visits.
