POSITION SUMMARY
The Care Coordinator provides person-centered care coordination and case management services to individuals receiving mental health services. This role ensures continuity of care by linking clients to appropriate behavioral health, medical, social, and community resources while supporting treatment goals, recovery, and stability. The Care Coordinator functions as part of the interdisciplinary treatment team and must maintainactive Qualified Mental Health Professional (QMHP) licensure in the Commonwealth of Virginia.
ESSENTIAL DUTIES & RESPONSIBILITIES
Care Coordination & Service Delivery
- Coordinate and monitor mental health and supportive services for assigned clients.
- Collaborate with clients to identify needs, goals, strengths, and barriers to care.
- Assist clients with accessing behavioral health, medical, housing, employment, educational, and social services.
- Facilitate referrals and ensure follow-up with service providers.
- Advocate for clients to promote continuity of care and reduce barriers to treatment.
Treatment Team Collaboration
- Participate in interdisciplinary treatment team meetings.
- Communicate regularly with clinicians, supervisors, and external providers.
- Support implementation of individualized service plans (ISP) and treatment plans.
- Provide updates on client progress, challenges, and service outcomes.
Documentation & Compliance
- Complete accurate, timely, and compliant documentation in accordance withDMAS, DBHDS, and agency policies.
- Maintain client records including assessments, service coordination notes, and progress documentation.
- Ensure services are delivered within scope of QMHP role and licensure.
- Comply with HIPAA, 42 CFR Part 2 (when applicable), and confidentiality standards.
Client Support & Engagement
- Provide psychoeducation related to mental health conditions, treatment options, and recovery supports.
- Assist clients in developing self-advocacy and independent living skills.
- Support crisis prevention planning and coordinate crisis services as needed.
- Maintain therapeutic boundaries and professional conduct at all times.
