Job DescriptionREPRESENTATIVE DUTIES
The following duties are typical for the Maternal Care Coordinator. Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.
Care coordination:
- Ensures Hamakua-Kohala Health is providing high quality care services.
- Assist patients through the healthcare system by acting as a patient advocate and navigator.
- Works in collaboration with providers and nursing staff, patients and their families to:
- Promote timely access to appropriate care
- Increase utilization of preventive care
- Reduce emergency room utilization and hospital re-admission
- Connect patients to relevant community resources.
- Facilitate transition from emergency room and hospitalization to follow up appointment at Hamakua-Kohala Health.
- Manage databases of assigned insurance carriers to identify existing care gaps and promote patient compliance.
- Participates as part of the team for data collection, health outcomes reporting, clinical audits, identification of gaps in care, and programmatic evaluation related to Care Coordination.
- Participate in Patient-Centered Medical Home quality improvement initiatives.
Specifics for Maternal Health:
- Manages a caseload of prenatal patients. Coordinates services and provides case management.
- Facilitates education on infant care, breastfeeding, and postpartum health.
- Ensure patients complete required prenatal labs and obstetrical imaging.
- Assists patients post-delivery by obtaining hospital birthing records, recording Apgar scores, and birth weights, assisting with FLMA and maternity leave paperwork, and conducting follow-up outreach to encourage postpartum appointments for 1 year postpartum.
- Facilitates referrals for clinically indicated services outside of HKH.
- Conducts outreach and supports patients, including home visits.
- Track the maternal patients in the AZARA OB module.
- Performs other duties as assigned.
EDUCATION, EXPERIENCE & QUALIFICATION GUIDELINES
Any combination of education and experience that would likely provide the required knowledge and abilities is qualifying
- High school graduate or equivalent. Must possess extraordinary customer service skills. Must have excellent organizational skills and ability to multi-task. Must be able to use a computer, type and comprehend computer logic.
- Valid Hawaii driver’s license and current auto insurance required.
- Pregnancy options counseling, family planning health worker training, breastfeeding education and home visiting training as identified by HKH needs to be completed within the first year of employment.

- Able to utilize Microsoft office applications, video teleconferencing platforms and is able to learn new skills in electronic documentation.

Working Conditions: Medical office environment and home visiting environment. Multi-task oriented position including direct patient care may require sitting or standing for long periods. Some bending, stooping, and lifting (up to 20lbs). Must interact with physicians, patients and other office personnel. Must possess socio-economic and cultural sensitivity. May involve contact with angry, upset or ill persons. Will involve potential exposure to blood and body fluids and other hazardous substances. Must have visual acuity and manual dexterity to assist with procedures, perform and interpret lab results, and interface with computer. Must have auditory acuity to handle phone calls and extensive patient interaction.
