Job Responsibilities:
- Designs and oversees a clinical program to ensure effective and comprehensive care delivery to clients.
- Participates in the creation, review, and revision of organizational policies and procedures.
- Communicates agency policies clearly to staff, clients, families, and external partners as needed.
- Ensures comprehensive client assessments are completed upon admission.
- Verifies that client admissions align with established agency criteria and policies.
- Oversees the development and recommendation of individualized care plans for all clients receiving services.
- Ensures continuity of care by monitoring the implementation of established care plans.
- Coordinates all client services to ensure they are delivered safely and appropriately.
- Monitors staff compliance with all relevant federal, state, and local regulations, as well as accreditation standards.
- Ensures access to necessary professional and ancillary services for clients, as appropriate.
- Provides nursing coverage in the absence of staff nurses, as needed.
- Maintains client records in accordance with local, state, and federal requirements, as well as accrediting body standards.
- Supervises nursing and paraprofessional personnel, offering guidance and support as necessary.
- Leads initiatives in quality assessment, quality program management, and continuous improvement.
Job Qualifications:
- Active and unrestricted Registered Nurse (RN) license and registration in New York State.
- Bachelor’s degree in Nursing (BSN) required.
- Minimum of three (3) years of clinical supervisory experience.
- At least one (1) year of experience in a home care setting.
- Valid New York State driver’s license and reliable access to a vehicle.
