Job DescriptionJob Overview/DescriptionThe Director of Nursing (DON)/Supervising Nurse, appointed by the Administrator, is responsible for the daily direction of patient/client care and participates in activities relevant to professional services furnished. The DON/Supervising Nurse and Alternate must be a registered nurse (RN) licensed in Texas or in accordance with the Texas Board of Nursing rules for Nurse Licensure Compact (NLC). Minimum of 3 years experience as a supervising Nurse/DON in Geriatrics Home Health Agency. The DON/Supervising Nurse must be able to perform the following job description:

Responsibilities

  • Able to perform PRN admissions, recertifications, resumption of care and field supervision when field nurses are unavailable.
  • QAPI (documentation, meetings, regulations): Participating in the development and implementation of the Quality Assessment Performance
  • Improvement Program: Collaborating with agency leadership and appropriate staff to develop the organizational plan as it pertains to patient/client care.
  • Collaborating on the development and implementation of policies and procedures, which guide and support the provision of care.
  • Performing annual emergency preparedness exercises
  • Performing and Maintaining PIP audits
  • Monitoring and ensuring that a patient’s/client’s Plan of Care or care plan is executed as written and visit notes are done accurately and turned in a timely fashion
  • Chart audits: Assuring the development, implementation, and updating of the individualized plan of care (includes making sure all documents are signed, following up on those not signed)
  • Monitoring quarterly IQIES reports
  • Works well with Axxess Agency Care Software and good understanding of CMS (Palmetto GBA) RCD on Pre claim review (PCR) performed and Axxess Care.
  • RCD documentation and uploads (making sure all paperwork required for RCD is ready and following up on those pending prior to submission, with goal of 90% approval)
  • Coordination of care with PT /OT /ST: Ensuring the care offered by the Agency, including any branches, is consistently available to all patients/clients.
  • Assisting in acquiring PT /OT /MSW referrals for patients it remote areas
  • Running patient/staff interference
  • Ensuring all physicians assigned to patients are signing and sending all required paperwork in a timely manner
  • Making sure visit notes are up to date and frequency is on track with LUPA requirements
  • Emergency Preparedness folder is up to date with State regulations and forms are filled out with current information and dates
  • Ensuring that Infection control is appropriately filled out on all patients on antibiotics, patient reviews are sent in, employee folders meet requirements, and employee license verification and files are audited
  • Performing employee evaluations
  • Directing and enforcing staff education, staff meetings/in-service, and staff assignments to meet State requirements/regulations
  • Keeping up with changes in CMS requirement and policies and making sure Agency implements and updates their policies to accommodate changes
  • Preparing for State Survey
  • Being available to Agency personnel at all times in person or by telephone

Oversee nursing operations, ensuring compliance with healthcare regulations and standards.

  • Develop, implement, and evaluate nursing policies and procedures to enhance patient care quality.
  • Manage nursing staff, including recruitment, training, performance evaluation, and professional development.
  • Collaborate with healthcare professionals to create individualized care plans for patients.
  • Monitor patient outcomes and implement improvements based on data analysis and feedback.
  • Ensure effective communication between nursing staff, patients, families, and other healthcare providers.
  • Maintain knowledge of current trends in nursing practices and healthcare regulations.
  • Foster a positive work environment that encourages teamwork and professional growth among nursing staff.

Skills

  • Strong background in home health and acute care settings.
  • Proven experience in nursing administration and nurse management.
  • In-depth knowledge of anatomy and physiology relevant to patient care.
  • Ability to manage multiple tasks effectively while maintaining attention to detail.
  • Excellent communication skills for interacting with patients, families, and healthcare teams.
  • Proficient in developing care plans tailored to individual patient needs.
  • Familiarity with healthcare systems and best practices in home care delivery.
