Your Role as a Home Health Physical Therapist
As an essential member of our team, your responsibilities will include:
- Providing skilled physical therapy according to personalized treatment plans created in partnership with referring physicians.
- Assessing patient needs and establishing rehabilitation objectives to improve mobility, strength, balance, and overall physical capabilities.
- Modifying treatment plans as needed to ensure optimal outcomes for your patients.
- Working alongside a multidisciplinary home health team, including nurses, occupational therapists, speech therapists, and social workers, to guarantee cohesive care.
- Instructing patients and caregivers on exercises, safety measures, mobility techniques, and the use of assistive devices.
- Accurately and promptly documenting all visits, evaluations, and progress notes.
- Empowering patients to enhance their independence and elevate their quality of life—no matter where they reside.
About You: Qualifications & Skills
We seek a candidate who possesses:
- A Master’s degree in Physical Therapy from an accredited institution.
- A valid PT license in the state of employment (or eligibility to obtain one).
- CPR certification.
- Excellent communication, assessment, and problem-solving abilities.
- The capability to work autonomously in diverse home settings.
- A valid driver’s license, auto insurance, and dependable transportation.
Preferred Experience (Not Mandatory)
- Over one year of experience in a hospital, home health, or outpatient rehabilitation environment.
- Experience in delivering therapy in home settings.
- Background in geriatric rehabilitation, orthopedic, or neurological rehabilitation.
- Experience in post-acute care or chronic condition management.
- Collaboration within a mobile or interdisciplinary care team.
