Full-Time | Days | 40 Hours per Week
Schedule: Monday–Friday, 8:30 AM–5:00 PM

Position Overview
We are seeking compassionate and skilled Registered Nurses (RNs) to provide high-quality home health care to patients in their homes. This role offers the opportunity to build meaningful one-on-one relationships with patients while delivering individualized nursing care in a comfortable and supportive environment.
The RN is responsible for assessing, planning, implementing, coordinating, and evaluating patient care in accordance with physician orders, professional nursing standards, regulatory requirements, and organizational policies.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing services in the home setting, including treatments, procedures, medication management, and patient education.
- Ensure care is delivered in accordance with physician orders and established standards of practice.
- Monitor patient progress and update care plans based on changes in condition.
- Perform medication reconciliation and collaborate with physicians regarding patient needs and treatment plans.
- Educate patients and caregivers using appropriate teaching strategies to support health goals and disease management.
- Coordinate care with physicians, therapists, home health aides, and other members of the interdisciplinary team.
- Maintain accurate, timely, and compliant clinical documentation.
- Identify and communicate patient care concerns, utilizing appropriate clinical judgment and critical thinking.
- Manage authorizations and ensure compliance with payer requirements.
- Supervise home health aide staff, including required supervisory visits and evaluations.
- Participate in care transitions and communicate patient information effectively to ensure continuity of care.
- Maintain productivity standards and complete assignments independently and efficiently.
- Stay current with nursing best practices, regulatory requirements, and evidence-based care guidelines.
- Perform other duties as assigned.
QualificationsRequired
- Associate Degree in Nursing or Diploma in Nursing
- Active New Jersey Registered Nurse (RN) License
- AHA BLS (Basic Life Support) Certification
- Minimum of six (6) months of RN experience or one (1) year of LPN experience
- Strong computer and documentation skills
- Valid driver's license and reliable transportation
Preferred
- Bachelor of Science in Nursing (BSN)
- Home Health, Hospice, or Community Health experience
Physical Requirements
- Ability to lift, push, and pull up to 50 pounds
- Ability to stand, walk, and perform patient care activities throughout the workday
