Pay: $2,325/week
Job Description
We are seeking a compassionate and experienced Physical Therapist (PT) to join a Long-Term Care / Skilled Nursing Facility (LTC/SNF) in Franconia, NH on a 13-week travel assignment. The PT will work with residents to improve mobility, manage pain, and enhance overall quality of life through individualized rehabilitation programs. The ideal candidate has strong clinical skills in geriatric therapy, excellent communication abilities, and a passion for delivering patient-centered care.
Key Responsibilities
- Evaluate residents’ physical conditions, mobility limitations, and rehabilitation needs to create customized treatment plans.
- Provide therapeutic exercises, gait training, and functional mobility interventions to improve strength, balance, and independence.
- Collaborate with physicians, nurses, occupational therapists, and interdisciplinary team members to coordinate care.
- Monitor progress and adjust therapy plans as needed to ensure optimal outcomes.
- Educate residents and their families on home exercise programs, adaptive equipment use, and safe mobility techniques.
- Maintain accurate and timely documentation of assessments, treatments, and outcomes in the medical record.
- Adhere to infection control practices, safety standards, and evidence-based rehabilitation care.
Qualifications
- Current and valid Physical Therapist (PT) license in New Hampshire (or eligibility to obtain).
- Minimum 1–2 years of recent experience as a PT in a LTC/SNF or geriatric care setting preferred.
- Graduation from an accredited Physical Therapy program.
- Current Basic Life Support (BLS) certification.
- Strong clinical assessment, communication, and problem-solving skills.
- Compassionate and patient-focused approach to rehabilitation care.
- COVID-19 vaccine and recent tuberculosis test.
