Job Summary:
Onero Rx is seeking a Medical Assistant (MA) to coordinate and support our Remote Patient Monitoring and Chronic Care Management program. This is an in-person role at our corporate location in West Des Moines, Iowa. The role is innovative healthcare initiative enables the remote tracking of patients’ health through cellular-connected blood pressure and blood glucose monitors. The CMA will lead engaging eligible patients, patient enrollment, coordinate Telemedicine visits, and manage ongoing patient monitoring tasks—playing a vital role in improving patient outcomes.
Key Responsibilities:
Patient Recruitment, Enrollment & Onboarding:
- Serve as a primary coordinator recruiting existing patients into clinical programs
- Coordinate initial patient visits at Onero Rx affiliated pharmacy locations.
- Confirm patient eligibility through insurance verification and documentation.
- Register patients and complete patient intake and monitoring device setup.
- Educate patients on the program, its health benefits, and proper device use.
- Facilitate a live Telemedicine visit between the patient and a remote physician during their visit.
- Collaborate with the pharmacist for a brief introduction at the start of each new patient visit.
Ongoing Monitoring & Care Management:
- Review patient-submitted vital signs daily via the RPM portal.
- Identify abnormal readings and escalate clinical concerns to pharmacists or providers.
- Perform regular care management including patient communication, record updates, and device troubleshooting.
- Ensure monthly monitoring compliance and follow-up scheduling.
Program Workflow Implementation:
- Utilize standardized workflows, task lists, and training protocols for optimal program execution.
- Maintain accurate records and support quality assurance efforts.
- Assist with data collection related to patient outcomes and program performance.
Program Impact:
- Contribute to meaningful patient outcomes by helping patients control their chronic conditions.

Required Qualifications:
- Experience in outpatient care, chronic disease management, or care coordination.
- Excellent communication and patient engagement skills.
- Comfortable using Telemedicine platforms and clinical monitoring software.
- Organized and able to work independently while managing multiple priorities.
Preferred Qualifications:
- Experience with Remote Patient Monitoring or population health management.
- Familiarity with working across virtual teams.
- Certified Medical Assistant (CMA) with active certification preferred.
