JOB TITLE: NURSE PRACTITIONER JOB CODE: 317/312

REPORTS TO: VICE PRESIDENT- CNO - EXECUTIVE ADMINISTRATION

OVERTIME ELIGIBILITY: Hourly (non-exempt) or Salaried (exempt)

OSHA CLASSIFICATION: OSHA Category I - employee is required to perform tasks that may result in occupational exposure on a regular basis.

OSHA RESPIRATORY CLASSIFICATION: OSHA Respiratory Category I - employee is required to enter isolated patient rooms.

CAREGIVER STATUS: Direct Caregiver. A Caregiver is an employee who provides direct patient care, hands-on care, or has skin-to-skin contact with patients.

JOB SUMMARY:

• The Nurse Practitioner is a professional Registered Nurse with advanced education and training in the primary care of adult and pediatric populations.

• The Nurse Practitioner is responsible for providing primary care to patients acting in collaboration with the Medical Director and as directed by established clinical protocols, policies and procedures.

QUALIFICATIONS:

JOB SPECIFIC CORE COMPETENCIES:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

• Current Indiana nursing license with renewal required.

• A Master’s of Science Degree in Nursing preferred.

• Minimum of two years of staff nursing experience required.

• One to three years of experience in family practice required.

• Professional knowledge of nursing theory and practice.

• Extensive knowledge of physical assessment, differential diagnosis,

pathophysiology, pharmacology and patient care management.

• Maintains CCU's for certification renewal.

EXPECTED BEHAVIORS:

• Demonstrate competency in family practice assessment, diagnosis, planning,

and prescribing.

• Excellent customer service skills.

• Emotional stability to cope with emergency situations.

MANDATORY LICENSE/REGISTRATION/CERTIFICATION:

The following licenses/certifications are required (if checked):

CPR (Cardio-Pulmonary Resuscitation)

ACLS (Advanced Cardiac Life Support)

PALS (Pediatric Advanced Life Support)

NRP (Neonatal Resuscitation Program)

CERTIFICATION/LICENSE: Nurse Practitioner (NP); Registered Nurse (RN)

ADDITONAL LICENSURE/CREDENTIAL REQUIREMENTS:

• Current State of Indiana prescriptive privileges or eligibility therefore.

• RN License.

• NP license.

• CSR license.

• DEA license.

• National NP certification required.

ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS: Mandatory

Continuing Education: Customer Service, Fire and Safety, Corporate Compliance

(including Confidentiality), Infection Control, and education required by regulatory,

accreditation bodies, scope of practice, and/or Hancock Regional Hospital.

WORK CONDITIONS

PHYSICAL/MENTAL DEMANDS:

• Assist ambulatory patients independently.

• Sitting up to one hour of day for computer documentation, written reports.

• Walking up to one hour of day.

EQUIPMENT USED: Equipment necessary to treat patients.

ENVIRONMENTAL CONDITIONS: Primarily outpatient clinic/office setting.
