Overview
Our growing home care agency seeks a Client Care Coordinator to join the team. You will work collaboratively clients, families, caregivers, and referral partners while helping ensure high-quality care and exceptional customer service.
Responsibilities
Client & Family Relations
1. Serve as the primary point of contact for clients, family members, and responsible parties throughout the care relationship.
2. Conduct in-home, non-medical care assessments for new and prospective clients, educating them on our agency's services and benefits.
3. Develop and maintain individualized plans of care tailored to each client's needs, preferences, and goals.
4. Conduct supervisory visits and scheduled reassessments to document any changes in client conditions or needs and ensure client satisfaction.
5. Communicate proactively with families regarding care updates, schedule changes, and any concerns.
Caregiver Scheduling & Coordination
1. Coordinate and manage caregiver schedules based on client needs, preferences, and geographic availability.
2. Match caregivers to clients thoughtfully, considering personality, skills, and continuity of care.
3. Promptly address scheduling gaps, call-outs, and coverage needs to ensure uninterrupted client service.
4. Serve as a key communication link between the office and field caregivers.
Compliance & Documentation
Assist with internal audits, external compliance reviews, and operational support requests as needed.
Qualifications
Required
Tremendous sense of urgency.
Superior organizational skills with the ability to manage multiple clients and priorities simultaneously.
Excellent interpersonal and communication skills. Warm, professional, and responsive.
Strong problem-solving abilities and attention to detail.
Minimum 3 years experience in home care, healthcare, or social services.
Valid driver's license, reliable transportation, and auto insurance.
Comfortable traveling throughout the service area (Chester, Delaware, and Montgomery Counties) for client and referral visits.
Proficient in Microsoft Office, Google Workspace, and basic computer systems.
Preferred
LPN or RN experience
Prior experience in a non-medical home care setting.
Experience with care plan development and supervisory visit documentation.
