TheCareCoordinatorworkswithamulti-disciplinaryteamincludingamentalhealththerapist,ayouth or family peer specialist, therapeutic recreation specialists, and education and healthcare professionals to engage and support families. This position provides a collaborative forum to engage a variety of child-serving partnersandadvocatefor theneedsofindividualsandtheirfamiliestosupportthefamilyremaining together and to improve the safety and satisfaction in their lives. This role provides direction in the developmentandimplementation ofcross-systemcare plansthataredesigned tohelptheyouth andfamily avoid out-of-home placement, end multi-system involvement, and incorporate additional resources and supports as necessary.
Qualifications:
- HighSchoolDiplomarequired,BAincounselingorrelatedfieldpreferred.
- RegisteredAgencyAffiliatedCounselorCredentialorabilitytobecomeregisteredwithin60daysrequired.
- Comprehensiveknowledgeofcommunityresourcesandpublicsystems.
- Minimumthreeyearsofworkingwithyouthandfamiliespreferred.
- Exceptionalcommunicationandinterpersonalskills.
- CommunityHealthWorkercertificationpreferred.
- Musthavestrongmultitaskingandorganizationalskills.
- Abilitytoworkinafast-pacedenvironmentandmakeethicalandsafedecisions.
- Abilitytomanageindividualstressandperformessentialjobfunctionsinasafeandprofessionalmanner.
- Mustbeabletopassacriminalbackgroundcheck.
- Mustbe21yearsorolder.
- Validstatedriverslicense.
Responsibilities:

- AdherestoWISeprogramtimelinesforcompletionofcarecoordinationtasks.
- Participatesintheon-callrotationtoensurefamilieshaveaccesstocrisisservices24/7.Thismay include phone support and in-person response.
- Providesdirectioninthedevelopmentandimplementationofcommunityresourceprogramsto achieve desired outcomes for children and families.
- UnderstandsandactivelypromotesWISeprogramsthroughoutthecounty,identifyingwaysin which efforts aimed at improving outcomes for children and families can be coordinated.
- Recognizesbarriersthatoccurinasystemofcareanddevelopandimplementpractices necessary to sustain system of care improvements.
- AssessesopportunitiesandbarrierstoachieveWISeservicesacrossallsystems.
- Developslongandshort-termgoalsandobjectivesforthedevelopmentandimplementationof agencies WISe program services across all systems of care.
- Seeksopportunitiestoeffectivelycommunicatewiththepublicandprivatechildandfamilyserving agenciesto share information about the program.
- ServesasliaisonbetweenallchildandfamilyservingagenciesandtheWISetreatmentteam.
- Coordinatescross-agencycommunicationandplanningasneeded.
- Usescompanyvehicleswhentransportingclients/familiestoprogramactivities.
- Completeallrequiredtraininganddemonstratescompetencyinskillsnecessaryforrole.
- IsawareofandavailabletoallWISeservicesasneededforadditionalsupportinperiodsofcrisis.
- Maintainsprofessionalismandunitywhencommunicatingwithclients,theirfamilies,andother representatives;preserves client confidentiality at all times.
- AttendsandactivelyparticipatesasarepresentativeofExcelsioranditssubsidiariesinmeetings, both internal and external.
- EffectivelyrepresentsExcelsiorinthecommunity,toincludebutnotlimitedtotheCityofSpokane, the State of Washington and other venues as attended.
- Adherestoorganizationalethicalstandards,policyandprocedures,mission,vision,andvalues.
- Thispositiondescriptionisnotintendedtobeall-inclusive,andtheemployeewillalsoperform other reasonably related business duties, across other Excelsior subsidiaries, as assigned.
