Title: Registered Nurse (Field RN) – Home Care / Waiver Program
Location: Yonkers, NY
Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Salary: $90,000–$100,000+ (flexible based on experience)
Mileage Reimbursement: Reimbursed at the current government mileage rate
Territory
The cases are mainly in Yonkers and lower Westchester County.
Any location from White Plains and south may be included within the assigned territory.
Position Overview
We are seeking a Registered Nurse (RN) to oversee patient care and supervise Home Health Aides in a community-based setting. This position serves individuals enrolled in waiver programs, including patients with Traumatic Brain Injury (TBI) and dementia.
This is primarily a supervisory and care coordination role. The RN is responsible for overseeing care delivery, conducting supervisory visits, managing documentation, and ensuring compliance with program requirements. This role does not involve routine hands-on bedside nursing care.
Key Responsibilities
- Manage an assigned caseload of approximately 26 patients.
- Conduct required supervisory visits for patients according to waiver program requirements.
- Supervise and support Home Health Aides providing care in the community.
- Perform Home Health Aide supervisory visits and evaluations.
- Conduct Home Health Aide training sessions at the office several times per month.
- Complete assessments, care plan reviews, and required documentation.
- Coordinate care with patients, families, caregivers, physicians, and interdisciplinary team members.
- Monitor patient status and ensure quality of care.
- Maintain compliance with agency, state, and waiver program regulations.
- Attend orientations, trainings, and meetings as required.
Caseload & Visit Expectations
- Assigned caseload of approximately 26 patients.
- Patients require supervisory visits every three months.
- Visit schedules are determined by due dates, patient needs, and other assigned responsibilities.
- There is no set number of visits required per day. Nurses are expected to effectively manage their caseload and complete all required visits within regulatory timelines.

Work Environment
- Combination of field visits and administrative work.
- Travel to patient homes is required.
- Significant documentation and paperwork are involved due to waiver program requirements.
- Documentation may be completed remotely, with periodic attendance at the office for orientation, aide training, and meetings.
Qualifications
- Active New York State Registered Nurse (RN) license.
- Home care, case management, community health, or waiver program experience preferred.
- Experience working with TBI and/or dementia populations is a plus.
- Strong organizational, communication, and documentation skills.
- Ability to work independently and manage a field-based caseload.
- Reliable transportation and willingness to travel throughout the assigned territory.
Compensation & Benefits
Competitive salary: $90,000–$100,000+ (flexible for strong candidates) and Benefits
