Social Worker (LCSW/LMFT) - San Jose, CA
Schedule: Full-time/Permanent

Job Summary:
Licensed Clinical Social Worker collaborates with clinic management, nursing, and clinician staff to maintain high standards of patient care with the patient at the center. The Licensed Clinical Social Worker independently provides social work evaluation and treatment services to at-risk client's and families with psychosocial needs as needed in general pediatric, adolescent and family medicine clinics; takes a leadership role in coordinating services with the interdisciplinary team and community agencies to ensure appropriate client care; advocates for and provides quality health care services to all persons in a culturally competent manner. The scope of service is directive in nature: to identify and manage clients with medical and mental/ behavioral health needs, provide target treatment, and monitor "at risk," "high risk," clients with a focus on the development of problem-solving and self-management skills as well as teaching coping mechanisms. Problems for which clients may be seen include: non-compliance with treatment plans (i.e., diet, exercise, and medications), academic problems (i.e., ADHD), mental health problems related to family disruptions and losses, peer related difficulties, physical /substance abuse and issues surrounding reproductive health and confidentiality.
Responsibilities and Duties:

Provide counseling and intervention to pediatric/adolescent clients; conducts individual therapy.

Performs a psychosocial needs assessment of clients and families.

Provide early detection of "at risk" patients, with the aim of preventing further psychological or physical deterioration.

Participates in treatment team, staff, and professional meetings to discuss client progress.

Develops implements and evaluates service plans based on identified needs.

Collaborates with client, family, clinic team members, school staff, primary care provider and community service organizations; maintaining patient confidentiality to ensure service plan and goals are integrated with needs.

Assists the organization to develop resources, communication, and impact in areas pertaining to mental health.

Assist with coordination of grant programs with the manager.

Comply with agency, program, and professional policies and procedures.

Attend meetings and training as well as represent the agency, as assigned by the supervisor and as required by the programs.
Qualifications and Skills:
-
Licensure by the State of California as a Licensed Clinical Social Worker or LMFT with pediatric/adolescent client experience.

Spanish speaking preferred

Masters Degree in Social Work from an accredited school.

A minimum of two years work experience in a health care agency or clinical setting, including case management, outreach, and education providing mental health services, preferred
