We are seeking a compassionate and skilled RN/LPN Clinical Liaison to join our team. This role involves working closely with patients, healthcare professionals, and the community to provide expert clinical guidance and support in wound care management. The Wound Care Clinical Liaison will act as the key point of contact for wound care services, promoting best practices in wound healing, educating staff, and ensuring the implementation of high-quality care standards. Will need to be able to travel overnights as needed for this position.
Key Responsibilities:
- Clinical Expertise: Provide expert clinical advice and consultation to healthcare teams regarding wound care management, ensuring evidence-based practices are implemented.
- Patient Care Coordination: Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans for patients with complex wounds.
- Education & Training: Conduct educational sessions for clinical staff on wound care best practices, new techniques, and emerging wound care technologies.
- Patient Advocacy: Act as a patient advocate, ensuring they understand their wound care treatment options and providing support and resources throughout the healing process.
- Interdisciplinary Collaboration: Collaborate with multidisciplinary teams to ensure holistic care for patients, including physical nurses, and operational leadership.
- Community Outreach: Promote wound care services to the community, fostering relationships with external healthcare providers, clinics, and specialists.
Qualifications:
- Education:
- Registered Nurse (RN), Licensed Practical Nurse (LPN), or another relevant clinical background.
- Experience:
- Minimum of 2 years of clinical experience in Home Health/Wound Care.
- Proven experience in educating and training clinical staff in wound care practices.
- Proven Account Executive experience strongly preferred.
- Skills & Abilities:
- Strong clinical assessment and wound care management skills.
- Excellent communication and interpersonal skills for working with patients, families, and healthcare teams.
- Ability to effectively educate staff and patients on wound care treatments and prevention strategies.
- Proficient in electronic health records (EHR) and documentation practices.
- Strong organizational and time-management skills.
- Other Requirements:
- Ability to travel locally to various healthcare facilities or patient homes as needed.
- A passion for improving patient outcomes and promoting best practices in wound care.
Physical Requirements:
- Ability to perform assessments, including bending, reaching, and walking for extended periods.
- Must be able to lift and transport equipment and supplies up to 25 lbs.
Work Environment: This role may require working in various healthcare settings such as hospitals, outpatient clinics, or patient homes. The Clinical Liaison may also participate in outreach programs and educational sessions in community settings.
