Position Overview – Mental Health / Psychiatric Nurse (RN)

The Grand Rehabilitation & Nursing at Barnwell is seeking an experienced and compassionate Mental Health / Psychiatric Nurse to join our Behavioral Health Department. This specialized role supports residents with psychiatric or behavioral health needs and collaborates directly with our interdisciplinary team—psychiatrists, psychiatric nurse practitioners, counselors, social work, and nursing leadership.

The Mental Health / Psychiatric Nurse plays a key role in delivering comprehensive, person-centered mental health care within a skilled nursing setting. This position ensures accurate assessment, thoughtful intervention, clear communication, and a safe, supportive environment for our residents.

Mental Health / Psychiatric Nurse ResponsibilitiesClinical Care & Psychiatric Management

  • Assess residents’ mental health status—including mood, behavior, cognition, and psychiatric symptoms—and document findings thoroughly.

  • Administer psychiatric medications and treatments as ordered by psychiatrists or psychiatric NPs.

  • Monitor residents for side effects, adverse reactions, or changes in mental status, and report promptly to the provider.

  • Support and participate in crisis management, behavioral interventions, and de-escalation techniques.

  • Collaborate with psychiatric providers to implement and update individualized treatment plans.

  • Participate in care planning and maintain detailed documentation within the EHR.

Collaboration & Interdisciplinary Support

  • Work closely with nursing staff, social services, therapy, and administration to coordinate comprehensive mental health care.

  • Provide input during interdisciplinary care conferences regarding psychiatric needs and treatment progress.

  • Serve as a resource on psychopharmacology, behavioral strategies, and mental health best practices.

  • Educate residents and families about mental health conditions, treatment options, and coping skills.

  • Participate in monthly GDR (Geriatric Drug Review) meetings to review psychotropic usage.

  • Ensure psychiatric consultations and follow-ups are completed promptly and accurately.

  • Identify and help overcome barriers to residents accessing mental health services.

Counseling & Psychosocial Support

  • Support residents participating in individual or group counseling sessions.

  • Monitor and document behavioral interventions, psychosocial progress, and group activity effectiveness.

  • Encourage resident engagement, coping skill development, and adherence to treatment plans.

Crisis & Safety Management

  • Respond promptly and appropriately to psychiatric or behavioral emergencies.

  • Ensure the safety of residents and staff using approved de-escalation and crisis intervention techniques.

  • Document incidents, medication reviews, and condition changes accurately.

  • Communicate concerns to psychiatric providers, unit managers, and nursing leadership.

Documentation & Reporting

  • Complete accurate and timely assessments, progress notes, medication administration records, and behavioral documentation.

  • Participate in quality improvement initiatives related to behavioral health and psychiatric care.

Qualifications

  • Registered Nurse (RN) license in New York State

  • Psychiatric or mental health nursing experience strongly preferred

  • Certification or advanced training in psychiatric nursing a plus

  • Experience in long-term care, hospital, or behavioral health settings strongly preferred

  • Knowledge of psychopharmacology, mental health disorders, and behavioral interventions

  • Strong communication skills with the ability to educate residents, families, and staff

  • Ability to collaborate effectively with interdisciplinary teams

  • Knowledge of HIPAA regulations and resident rights in mental health care
