Home Health Registered Nurse (RN)


Location: Brunswick, GA

Schedule:

  • Monday–Friday, 8:00 AM – 5:00 PM
  • On-call rotation approximately once every three weeks
  • Hours are not guaranteed


Coverage Area:

  • Glynn County
  • Camden County
  • Brantley County
  • Charlton County


Mileage Reimbursement:

  • $0.43 per mile


Position Overview

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing services and care coordination for patients within a home health setting. This role is responsible for conducting comprehensive patient assessments, developing individualized care plans, coordinating services across the healthcare continuum, and promoting positive patient outcomes.

The ideal candidate will possess strong clinical judgment, exceptional communication skills, and a commitment to delivering high-quality, patient-centered care.


Key Responsibilities

  • Perform comprehensive assessments of high-risk patients and evaluate ongoing care needs
  • Develop and implement individualized care plans in collaboration with physicians and other healthcare providers
  • Coordinate patient care and facilitate smooth transitions between levels of care
  • Monitor patient progress and continuously reassess treatment effectiveness
  • Provide skilled nursing services, including wound care and IV therapy
  • Identify barriers to care and coordinate non-medical support services such as transportation and housing resources
  • Collaborate with physicians, social workers, discharge planners, and other healthcare professionals to ensure optimal patient outcomes
  • Engage specialty resources and community services when appropriate
  • Promote health education, disease prevention, and wellness initiatives
  • Document clinical, functional, and financial outcomes accurately and timely
  • Identify potential risks and intervene proactively to prevent adverse patient events
  • Serve as a patient advocate while maintaining privacy, confidentiality, and regulatory compliance
  • Meet established productivity expectations of 30 units


Required Qualifications

  • Current and unrestricted Georgia Registered Nurse (RN) license or Compact RN License
  • Associate's or Bachelor's Degree in Nursing
  • Previous Home Health nursing experience required
  • Experience with wound care and IV care
  • Proficiency with Homecare Homebase (HCHB) software
  • Strong assessment, critical thinking, and care coordination skills
  • Ability to analyze complex clinical information and make sound decisions
  • Excellent communication and interpersonal skills


Preferred Qualifications

  • Case Management Certification (CCM or equivalent)
  • Experience managing high-risk patient populations
  • Knowledge of community resources and care transition processes
