Location: Hunterdon, Somerset, Warren & Mercer Counties, NJ
Compensation: $100 Per Patient Visit
Schedule: Full-Time | Monday–Friday | Flexible Daytime Hours

We are seeking a compassionate and motivated Licensed Physical Therapist (PT) to provide one-on-one skilled therapy services to patients in the comfort of their homes. If you enjoy creating meaningful relationships, working independently, and helping patients regain mobility and independence, we'd love to hear from you!
Responsibilities:
- Conduct comprehensive physical therapy evaluations.
- Develop individualized treatment plans based on physician orders.
- Provide skilled therapeutic interventions to improve strength, balance, mobility, and functional independence.
- Educate patients and caregivers on home exercise programs and safety techniques.
- Accurately document patient progress using electronic medical records.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team.
- Participate in care planning and interdisciplinary case conferences.
Qualifications:
- Current New Jersey Physical Therapist License (Required).
- Minimum of 1 year of physical therapy experience (Home health experience preferred).
- Strong assessment, communication, and clinical documentation skills.
- Reliable transportation, valid driver's license, and current auto insurance.
- Current CPR certification.
- Ability to work independently while managing a flexible schedule.
