Annual Salary: $49,612

Work schedule: Monday - Friday 8:30 am - 5:00 pm

WHAT'S YOUR DAY LIKE AS A CASE MANAGER/SUPPORT COORDINATOR?

As a Case Manager/Support Coordinator, you will develop personalized care plans and connect individuals living with developmental disabilities to essential services and supports. Your role includes advocacy, counseling, crisis intervention, and daily living support to promote independence and inclusion in the community. Your responsibilities will include supporting these individuals' fundamental physical, financial, social, and personal requirements. From developing personalized care plans to facilitating connections with service providers, your work will be instrumental in promoting the well-being and community integration of those you serve. Additionally, you will provide ongoing monitoring, advocacy, crisis intervention, counseling, and daily living support to empower individuals living with developmental disabilities to lead fulfilling lives.

WHAT'S REQUIRED

In this role, you should have:

• Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.

• One (1) year of experience providing case management or support coordination services to adults living with a developmental disability.

• Valid Driver's license
