Job Overview
Client is seeking a dedicated Physical Therapist to provide high-quality outpatient rehabilitation services in Sacramento, CA. The successful candidate will evaluate patients, develop individualized treatment plans, and oversee assistants and aides to ensure optimal patient outcomes. This role emphasizes thorough documentation, collaboration with a multidisciplinary team, and adherence to state regulations and company policies. Outpatient experience, especially with orthopedic and musculoskeletal conditions, is preferred.
Key Responsibilities
- Evaluate and treat patients, creating individualized physical therapy treatment plans
- Maintain accurate documentation of goals, progress, and medical necessity
- Communicate effectively with patients, families, and healthcare team members
- Ensure compliance with state regulations, company policies, HIPAA, and safety standards
- Participate in ongoing professional development and maintain required certifications
- Support facility operations, workflow, and productivity goals
- Lead or assist in clinical in-services and team initiatives
Qualifications
- Graduate of a CAPTE-accredited Physical Therapy program with active state PT licensure
- Outpatient physical therapy experience preferred
- Proficient in clinical documentation systems and Microsoft Office
- Strong problem-solving, decision-making, and patient-care skills
- Ability to supervise assistants and aides effectively
- Excellent organizational, time management, and leadership skills
- Team-oriented, adaptable, and patient-focused
- Ability to lift and assist patients up to 50 lbs
- Frequent walking, bending, standing, and reaching required
- Visual and auditory acuity for patient evaluation and documentation
- Competent in operating therapeutic and mobility equipment
