As a Home Health & Hospice RN, you will:

  • Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas.
  • Utilize yourcritical thinking and assessment skillsto develop and implement individualized care plans.
  • Perform a range of essential nursing skills, includingwound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage.
  • ProvideLNA supervision.
  • Workindependentlywhile being a valued member of our collaborative team.
  • Maintain accurate and timely documentation.
  • Participate in arotating weekend and on-call scheduleto ensure continuous patient support.
  • Travel to patient homes in various weather conditions.

What You Need to Succeed:

  • Required:CurrentNew Hampshire Registered Nurse (RN) license.
  • Required:ValidDriver's Licenseandinsured vehicle.
  • Essential:Minimum ofone year of professional nursing experience.
  • Key Skills:Ability to workindependently, strongself-motivation, excellentorganizational skills, and the ability to travel.
  • Clinical Expertise:Demonstrated proficiency inwound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage.
  • Education:Associate's degree or higher in nursing.

Work Environment:

  • On the road:Traveling to patient homes within the designated service area.
  • Day shiftwith arotating weekend and on-call supplemental schedule.
