As a Home Health & Hospice RN, you will:
- Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas.
- Utilize yourcritical thinking and assessment skillsto develop and implement individualized care plans.
- Perform a range of essential nursing skills, includingwound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage.
- ProvideLNA supervision.
- Workindependentlywhile being a valued member of our collaborative team.
- Maintain accurate and timely documentation.
- Participate in arotating weekend and on-call scheduleto ensure continuous patient support.
- Travel to patient homes in various weather conditions.
What You Need to Succeed:
- Required:CurrentNew Hampshire Registered Nurse (RN) license.
- Required:ValidDriver's Licenseandinsured vehicle.
- Essential:Minimum ofone year of professional nursing experience.
- Key Skills:Ability to workindependently, strongself-motivation, excellentorganizational skills, and the ability to travel.
- Clinical Expertise:Demonstrated proficiency inwound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage.
- Education:Associate's degree or higher in nursing.
Work Environment:
- On the road:Traveling to patient homes within the designated service area.
- Day shiftwith arotating weekend and on-call supplemental schedule.
