About Our Facilities
Our organization operates Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily care.
- Short-term rehabilitation: Services for patients recovering from surgery, illness, or injury.
- Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.
As a therapy professional, you will play a vital role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-focused environment.
Key Responsibilities
- Provide direct rehabilitation care in speech therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.
- Participate in community health initiatives and projects as needed.
- Maintain proficiency in all competency areas, including patient rights, safety, and sanitation.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Ensure confidentiality of all proprietary and sensitive information.
- Demonstrate integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
- For SLP CCC holders: Oversee and evaluate care provided by SLP Clinical Fellowship Year (CFY) candidates.
- For SLP CFY candidates: Follow speech therapy treatment plans under the supervision of an SLP CCC, in compliance with federal and state guidelines.
- Utilize therapy software accurately and effectively.
- Exhibit excellent customer service and maintain a positive attitude towards patients.
Qualifications
- Graduation from an accredited speech therapy program (minimum MS or MA in SLP).
- ASHA certification (CCCs) or eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification.
- One year of experience in post-acute care or a related setting is preferred.
- Proficiency in Microsoft Word, Excel, and email communication.
- Ability to communicate effectively in English, both verbally and in writing.
