Then you may be the hero we’re seeking to provide nourishing meals and wellness checks to aging adults countywide and beyond lacking the access and/or the ability to dependably prepare their own healthy meals.

MOW YOLO APPROACH
Meals on Wheels Yolo County (“MOW Yolo”) is adding a Community Navigator (CN) to our already inspired team. We are looking for a CN who will:
● Support efforts with MOW Yolo seniors to identify their needs and connect them with appropriate resources and services.
● Prioritize nutritious food security and socialization so that seniors in the region may EAT: Eat, Age, Thrive.
● Positively represent MOW Yolo internally and externally.

POSITION SUMMARY:
The Community Navigator(CN) reports to the Community Health Worker (CHW) and provides compassionate, person-centered support to seniors, ages 60 and older. The work of the CN furthers MOW Yolo’s mission to nourish and engage vulnerable older adults and others with mobility challenges throughout Yolo County and beyond.

USUAL WORK SCHEDULE:
The CHW is a part-time (20 hours per week), non-exempt position working Monday through Friday, 9:00am-1:00pm with some flexibility available based upon organizational needs. Primary work location is in the field at MOW Yolo participant homes, congregate meal locations, and other sites throughout Yolo County, including the MOW Yolo Administrative office in Woodland and the MOW Yolo meal production facility in Winters.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
1. Participant Intake & Assessment: Assist with screening referrals; conduct welcoming, person-centered assessments to identify participant needs, barriers, and goals.
2. Participant Support & Coordination: Work collaboratively with participants and the CHW to identify strategies, provide support, and connect seniors with appropriate community resources and services.
3. Relationship & Community Engagement: Build positive relationships with seniors, caregivers, volunteers, colleagues, and community partners while participating in outreach and promoting MOW Yolo programs and services.
4. Documentation & Communication: Maintain accurate participant records and required documentation, communicate regularly with supervisors and colleagues, and fulfill mandated reporting responsibilities.
5. Professional & Program Responsibilities: Maintain knowledge of senior-focused community resources, attend staff meetings, adapt to evolving program needs, and perform other assigned duties.

QUALIFICATIONS:
1. Education & Experience: High school diploma or equivalent required; Experience working with seniors and knowledge of aging-related needs preferred; outreach experience desirable.
2. Physical Requirements: Ability to lift and carry up to 50 lbs., stand for extended periods, bend below the waist, and reach overhead.
3. Independent & Adaptive Work: Ability to work independently with minimal supervision and respond flexibly, creatively, and patiently to varied and unexpected situations.
4. Communication & Technology Skills: Ability to read, write, perform basic math, and accurately maintain records and reports. Proficiency with or willingness to learn electronic systems; Microsoft Office experience required, with Google Workspace familiarity preferred.
5. Driving & Professional Standards: Valid California Class C driver’s license with a satisfactory driving record, maintained in good standing. Demonstrates professional hygiene, appearance, conduct, and demeanor.

POSITION LICENSES/CERTIFICATIONS:
1. Current California Driver’s License.
2. Proof of current automobile insurance.
3. Mandated Reporter Training, Mental Health First Aid Cert, BLS, Motivational Interviewing, (MOW Yolo pays for this training if needed)

COMPENSATION AND BENEFITS
$23-$25 per hour (DOE), payable per the regular payroll schedule and subject to applicable withholding and payroll taxes and other deductions required by law. Part-time employees receive vacation and sick time accrual, 11 paid holidays annually, and the option to participate in a 401(k) Retirement Savings Plan.
This position is funded through a grant and is contingent upon the availability and continuation of grant funding.
