Clinical Care Registered Nurse - Battle Creek\nPrimary Purpose\nThe Clinical Care Registered Nurse (CCRN) role provides direct nursing care to participants. This role works independently in the application of advanced nursing knowledge and skills and is able to manage complex clinical situations. This role collaborates with both licensed and non-licensed personnel.\nKey Responsibilities\n\nTriaging emergent situations and able to remain independent in the application of advanced nursing knowledge.\nMonitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).\nUtilizing care plans independently to provide clinic and home-based direct care.\nAdministers treatments, procedures and medications as prescribed to participants.\nMakes home visits on a regular basis to provide care and treatment as ordered by the PCP. This may include wound care, lab draws, nurse assessments and other nursing tasks.\n\nQualifications\n\nBachelors degree preferred in Science of Nursing; Associates degree required.\nRNNursing Diploma or Degree from accredited School of Nursing or University.\nMinimum of one year of experience working with the frail, elderly, or long-term care population.\nMinimum of one year home care experience preferred.\nMinimum of one-year wound care experience preferred.\n\nSpecific Requirements\n\nPossess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners PACE\nDemonstrates our core values of compassion, integrity, teamwork, and excellence.\n\nWork Environment\n\nHas visible presence while working in the center and is subject to frequent interruptions.\nCommunicates with staff, participants, caregivers, and other organizations, as necessary.\nWorks beyond normal working hours and on weekends and holidays, as necessary.\n\nMinimum Working Requirements\n\nMust have reliable transportation, current valid Michigan drivers license and car insurance.\nMust be 18 years of age or older.\nMust be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.\n\n
