Pharmacist (Coverage)
Location: NHC Hawaii, HI
Schedule: Monday through Friday between 0600 and 1800
Shifts are normally 8.5–9 hours, including an uncompensated 30–60 minute lunch break
Will not exceed 80 hours per two-week pay period

Job Summary
The Pharmacist provides comprehensive pharmaceutical care within the scope of clinical privileges granted by Medical Treatment Facility (MTF) Leadership. This role delivers Medical Therapy Management (MTM) services to eligible adult and pediatric patients and collaborates with the healthcare team to optimize medication therapy, promote health maintenance, and support preventative care.
.Essential Job Duties and Responsibilities

Provide Medical Therapy Management (MTM) services for conditions including, but not limited to, anticoagulation, diabetes, asthma, hypertension, hypercholesterolemia, and chronic pain.

Coordinate and collaborate with physicians and other healthcare team members to conduct medication reviews and optimize patient outcomes.

Perform a full range of pharmacist procedures within the facility’s scope of services.

Deliver comprehensive pharmaceutical services consistent with facility personnel, supply, and equipment capabilities.

Counsel patients on the appropriate use of medications, including dosing, administration, and potential side effects.

Review patient medication profiles to monitor therapy effectiveness and safety at the time of dispensing.

Verify prescription accuracy, including directions for use, dosage, drug interactions, therapeutic duplications, allergy considerations, and age-appropriate dosing.

Intervene with prescribing providers when discrepancies or concerns are identified.

Compound medications as required.

Ensure accuracy of pharmaceutical products prepared or processed by pharmacy technicians.

Oversee pharmacy inventory control, including scheduled and controlled substances.

Maintain required records and ensure pharmacy security in compliance with all applicable laws and regulations.
Required QualificationsEducation
-
Baccalaureate Degree in Pharmacy OR Doctor of Pharmacy (PharmD)
(Degree must be from a College of Pharmacy accredited by the American Council for Pharmacy Education – ACPE)
Experience
-
Minimum of 1 year of experience within the past 3 years as a pharmacist in an outpatient or retail pharmacy setting
Licensure
-
Current, valid, unrestricted pharmacist license in any U.S. state, the District of Columbia, Puerto Rico, Guam, or the U.S. Virgin Islands
Certifications
-
Basic Life Support (BLS) required

No additional certifications required
