Key Responsibilities:

The Acute Registered Nurse (RN) will:

• Provide high-quality advanced acute care in a home setting.
• Deliver daily patient care that meets hospital-level standards.
• Conduct clinical assessments and home evaluations.
• Educate patients and families on care procedures.
• Document patient progress and treatment plans.
• Collaborate with interdisciplinary teams to coordinate care.

Qualifications & Experience:

• Education: Must be a graduate of an accredited nursing program.
• Licensure: Responsible for obtaining and maintaining a current RN license in the state of employment, adhering to all state board requirements.
• Experience: At least two years of nursing experience in an acute care or emergency department setting is required.
• Certifications: Must possess a Basic Life Support Health Care Provider card or complete the course within 30 days of hire.
• Additional Skills:
  • Strong nursing, communication, and organizational abilities for effective home visits.
  • Capability to work independently and adapt to changing situations.
  • Excellent interpersonal skills and the ability to work with diverse populations.
  • Proficiency in various computer software applications and technologies.
  • Willingness to travel within the designated geographic area.
