Position Responsibilities

• Conduct evaluations
• Develop treatment plans
• Deliver skilled physical therapy services
• Monitor patient progress
• Maintain timely documentation
• Educate caregivers
• Collaborate with physicians and interdisciplinary providers

Qualifications

Active Arizona PT or DPT license, CPR certification, AHCCCS ID (we can help you get this), Article 9 certification, dependable transportation, and strong interpersonal skills.

Pediatric and home health experience are welcomed but not required.
