Physical Therapist (PT) – Full-Time Pleasant Valley Nursing &amp; Rehab Center Derry, NH $45–$50/hour
Post Rehab is seeking a Physical Therapist to join our rehabilitation team in a skilled nursing setting. This role offers the opportunity to work with residents focused on improving mobility, strength, and overall functional independence.
Responsibilities:

<li>Perform physical therapy evaluations and establish individualized treatment plans</li>
<li>Provide therapy focused on gait training, transfers, balance, strengthening, and mobility goals</li>
<li>Collaborate with rehab and nursing staff regarding resident care plans</li>
<li>Monitor progress and update treatment approaches as needed</li>
<li>Maintain accurate and timely documentation</li>

Qualifications:
