Haven Health, the leading Home Health, Hospice, and Palliative Care company in Southern California, is currently hiring a Nurse Practitioner to join our Palliative Care team.

Responsibilities:
- Visit patients in the home and assess their plan of care
- Submit patient records and documentation in a timely manner
- Play an integral role on the Interdisciplinary Care Team
Requirements:
Qualifications:
- Master's Degree in Nursing
- Bilingual in English and Spanish
- Licensed to practice in the state of California
- Licensed and insured driver with reliable transportation
- Understanding and acceptance of hospice principles of care
- Comprehensive knowledge and well-developed skills in medicine, pharmacology, pain and symptom control, and patient admissions criteria
