Overview

WoundCare 360 Advanced Solutions is seeking an experienced and motivated Wound Care Nurse Practitioner to join our mobile clinical team. In this autonomous, field-based role, you will evaluate and manage patients with a variety of complex wounds, including pressure injuries, diabetic foot ulcers, venous leg ulcers, and surgical wounds. You’ll perform sharp debridement, apply advanced wound therapies, and create individualized treatment plans rooted in evidence-based best practices. The ideal candidate is a clinically skilled, patient-centered provider with a strong background in wound healing and a passion for improving patient outcomes in the home and facility-based care settings.

Key Responsibilities

  • Perform comprehensive wound assessments, including staging, measuring, photographing, and documenting in the EMR.
  • Provide advanced wound care treatments such as sharp debridement, enzymatic debridement, negative pressure wound therapy, and biologic applications (e.g., amniotic allografts).
  • Develop and implement evidence-based wound care plans tailored to individual patient needs.
  • Monitor wound healing progress and modify care plans as clinically indicated.
  • Order, review, and interpret labs, imaging, and other diagnostics relevant to wound healing and infection management.
  • Educate patients, families, and caregivers on wound care strategies, dressing changes, and prevention techniques.
  • Collaborate with referring physicians, PCPs, home health nurses, and facility staff to ensure continuity and coordination of care.
  • Maintain accurate, timely documentation in compliance with regulatory and organizational standards.
  • Travel to patient homes.

Experience

  • A Master's degree in Nursing or a related field, with certification as a Nurse Practitioner.
  • Proven experience in wound care management or a related clinical setting is required.
  • Strong knowledge of medical terminology, physiology, and patient assessment techniques.
  • Ability to lift patients or equipment as required for effective patient care delivery.
  • Excellent communication skills, both verbal and written, with the ability to teach complex concepts clearly.
