Position Overview

The Charge Nurse oversees nursing staff during shifts, ensures patient care quality, manages supplies and household operations, and handles immediate clinical needs. This role combines leadership with hands-on care, ensuring compliance with healthcare standards, maintaining documentation, managing daily operations related to patient care, and contributing to household organization and staff development.

Reports to: Director of Nursing (DON)

Key Responsibilities

  • Staff Supervision and Coordination
    • Monitor daily operations, ensuring efficient workflow and optimal allocation of resources.
    • Conduct staff reviews and provide ongoing training and mentorship to nursing staff.
    • Adjust and manage staff schedules in KanTime and post to Teams.
    • Post open shifts on agency job boards to ensure coverage.
    • Be on-call to provide immediate support or direction to on-duty staff during emergencies.
    • Generate talking points and concerns for monthly staff meetings.
  • Patient Care Quality Assurance
    • Conduct quality assurance (Q/A) checks on patient charts for compliance and accuracy.
    • Verify patient schedules and authorizations regularly.
    • Act as the escalation point for complex patient concerns and issues.
  • Medication and Order Management
    • Add new medications to patient records as necessary.
    • Update and maintain NARC (Narcotic) sheets.
  • Use OmniView to refill medications and ensure accurate documentation.
  • Patient and Household Supplies Management
    • Order and inventory DME, respiratory, food, household, and personal supplies.
    • Verify inventory received against packing slips and resolve discrepancies.
    • Oversee household cleanliness standards, organizing daily room checks and general house operations.
  • Transportation and Patient Logistics
    • Request transportation for patients and ensure smooth coordination.
    • Order and manage specific food needs, such as Bridget’s food.
  • Clinical Care and Support
    • Provide direct patient care as necessary, including administering medications, monitoring vitals, and assisting with treatments.
    • Serve as a clinical resource for nursing staff, addressing complex patient needs.
    • Perform APD Monthly Monitoring Visits and document findings.
  • Administrative and Communication Duties
    • Respond to voicemails, texts, and calls on the Charge Nurse cell phone.
    • Manage tasks in Microsoft Planner to ensure operational efficiency.
    • Regularly update and maintain Plan of Care and Standard Orders documentation.
    • Manage and distribute eFax files in Worldview.
    • Ensure the "Clip Board" is current with necessary information for staff reference.
  • Documentation and Reporting
    • Complete incident reports and escalate critical incidents to the DON.
  • Ensure accurate documentation of patient care activities, assessments, and plans.
  • Perform periodic checks on "Everything You Need" files to ensure staff have the necessary resources.

Required Qualifications

  • Education:
    • Associate or Bachelor’s degree in Nursing (BSN preferred).
    • Current, unencumbered Registered Nurse (RN) license in the state of operation.
  • Experience:
    • Minimum of 3-5 years of nursing experience, with at least 1-2 years in a supervisory or charge nurse role, preferably in a home health or similar setting.
    • Experience with electronic medical records (EMR) systems and proficient in documentation standards.
  • Skills:
    • Strong clinical knowledge and ability to provide expert guidance on patient care.
    • Proficiency in time management, critical thinking, and problem-solving.
    • Familiarity with healthcare regulations and quality assurance practices.
  • Certification:
    • BLS/ACLS certifications preferred.
