Position Overview
•\tLocation: Ortonville, MN
•\tSetting: OP; SNF/LTC; limited IP; HH; ALF/ILF
•\tPay Package: Starting at $1800/week
•\tGuaranteed Hours: 36-hour weekly guarantee
•\tSchedule: Tuesday–Friday, 8:00 AM–4:30 PM (No weekends, holidays, or call)
•\tContract Length: 13 weeks
•\tStart Date: 10/05/2026
Qualifications
•\tGraduate of an approved school of Physical Therapy, with a minimum of a bachelor's degree
•\tValid state PT licensure (or currently in a PT educational program)
•\tCPR certification
•\tValid driver’s license
•\tMust successfully pass a background check upon hire
•\tStrong communication and interpersonal skills
•\tExperience in SNF; OP; IP; HH; ALF/ILF preferred — new grads encouraged to apply!
Responsibilities
•\tProvide supervision for Physical Therapist Assistants and Therapy Aides in accordance with state/federal regulations
•\tEvaluate patients, review medical history, and diagnose movement dysfunction
•\tDevelop and modify individualized treatment plans
•\tTeach therapeutic exercises and provide hands-on techniques to support healing
•\tMaintain accurate documentation and patient progress records
•\tEducate patients and families on home programs and recovery expectations
•\tPerform administrative tasks including reports, communication, and documentation per HIPAA
