Job Summary:

As a Home Health Speech Therapist with Carevana Home Health, you will provide specialized speech and language therapy services to patients in the comfort of their homes. In this vital role, you will be responsible for evaluating, diagnosing, and treating a range of speech, language, communication, and swallowing disorders.

You will play a key role in developing individualized therapy plans, delivering effective and evidence-based interventions, and collaborating closely with patients, families, and interdisciplinary team members. Your work will directly enhance each patients ability to communicate effectively and improve their overall quality of life.

Job Qualifications:


  • Valid Speech-Language Pathologist (SLP) license in the state where work is performed.
  • 1 or more years of experience in speech therapy, preferably in a home health or similar setting.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.
  • Strong clinical skills and knowledge of speech and language therapy principles, techniques, and best practices.
  • Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
  • Ability to build rapport with patients through a compassionate and caring demeanor.


PRN - As available for weekdays and weekends


Primary Responsibilities and Duties:


  • Conduct thorough assessments of patients speech, language, communication, and swallowing abilities to identify deficits and develop individualized therapy plans.
  • Design and implement targeted therapy programs to address specific disorders, including speech articulation, language comprehension and expression, cognitive-communication skills, and dysphagia (swallowing difficulties).
  • Educate patients and their families on therapy techniques, home exercises, and strategies to support and enhance communication and swallowing functions.
  • Monitor and document patient progress, response to therapy, and any changes in condition, providing detailed reports to the supervising physician and other healthcare team members.
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated and holistic approach to patient care.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
  • Participate in team meetings to contribute to comprehensive care planning and decision-making.
  • Stay current with advancements in speech therapy practices and participate in ongoing professional development.
  • Perform all other duties assigned.
  • Keep abreast of speech therapy trends and knowledge through seminars, CEUs, self-study, and workshops.
  • Participate in in-service programs.
  • Prepare the patient and/or family for discharge as appropriate.
